Cancellation policy: - If you change or cancel an appointment, we require a minimum of 24-hour advance notice. Cancelling within 24 hours will result in a $50 cancellation fee.
Appointment lateness: - Please arrive 15 minutes before your scheduled appointment. If you arrive more than 10 minutes after your scheduled appointment, you may not receive the full time allocated for your treatment. If you arrive 20 minutes after your start time, you may be asked to wait until the next available opening or to re-schedule.
Medical records: - We keep a copy of the health care services provided to you. We will not disclose your records to others without a signed medical release from you or unless the law authorizes or compels us to do so.
Cell Phones: - Please set phones to vibrate or silent when in the therapy area.
Minor Children: - Children under eighteen years of age receiving therapy must be accompanied by an adult guardian for their initial evaluation. Follow up treatments do not require a parent or guardian to be present, provided a "Consent to Treat a Minor" document has been completed. Children who are not receiving therapy are not allowed in the therapy area.
