Your First Visit

At METRO SPORTSMED® we welcome you as an individual with distinct needs that require a unique, customized program of care. We consider ourselves partners with you on your road to recovery and wellness.

For your first visit, you can expect a one-on-one examination so that we can determine the best program to relieve your discomfort and restore your normal level of activity.

Please wear loose, comfortable clothing and be aware that your first treatment may take place at this time.

We ask that you come 30 minutes earlier than your scheduled time with the therapist so we can complete the required documentation.

We will need the original prescription or referral from your physician, a copy of your insurance card (front and back), a photo ID that we can make a copy of, and have you verify your insurance benefits information. We will also collect any co-pays and deductibles as required by your insurance company.

At this time as well we will ask that you read and acknowledge our company policies as well as the HIPAA Notice of Privacy Practices (both of which can be downloaded from this website.)

We also ask that you complete a medical history document so that your therapist can be up to date on your health status. Finally, you will be asked to sign a Consent for Care and Treatment Form.

We try to keep the paperwork to a minimum so that you can quickly proceed towards your treatment and recovery. The paperwork we ask you to provide is necessary and in many instances, is mandated by law.

We will be glad to answer any questions that you have at any time.

Please be sure to bring these things to your first appointment:

• Your original prescription
• A copy of your insurance card
• Photo identification
• List of prescription and over-the-counter medications you are taking
• Shorts or comfortable clothing.
